The new Advance Decision to RefuseTreatment (which includes the redesigned and updated Advance Statement of Wishes) is now available from USS.

The document is free. 


The ADRT (and ASOW) allows you to record details of any treatment you would wish to refuse should you become incapacitated at some time in the future and also your wishes for how basic personal affairs should be dealt with should you become poorly.

Health professionals may ask if you have one.
